Position Summary

The primary purpose of the case manager position is to assist Apex residents and ACH community who have had recent police involvement. Through the role as a case manager, you will support residents/ patients connect to community-based resources to address mental and physical health needs. Case managers will also support with addressing social determinants of health and connection to community resources. The goal of this position is to decrease patients’ future police contact.

Essential Duties and Responsibilities

• Aid the Apex Police Department Crisis and Advocacy Response Team (CART) and Victims Advocate with wraparound care for community members.
• Provide community resources and referrals to medical facilities, social service agencies, and other appropriate services.
• Conducts follow-ups of all criminal incident reports and Computer Aided Dispatch (CAD) calls for non- intimate partner domestic violence related incidents or any mental health crisis calls; provides information and assistance and researches.
• Promotes integration of services for members including behavioral health care and long-term services and supports to enhance the continuity of care.
• Assesses for barriers to care, provides care coordination and assistance to member to address concerns.
• Knowledge of the legal and ethical issues relative to confidentiality of client records and the ability to interpret and apply them within the scope of the case management responsibilities.
• Serve as a liaison between the Apex Police Department and Advance Community Health and local mental health agencies and hospitals, social and human services agencies, emergency services providers and community groups with social services and crisis intervention resources.
• Independently conferences with treating providers at Advance Community Health.
• Complete and accurately update all documentation in a timely manner as required.
• Demonstrable ability to work independently and to make sound judgments regarding client care.
• Work collaboratively with all members of the treatment plan as often as needed.
• General knowledge of community and social service resources for mental health, substance use, domestic violence, etc.

Education and Experience

• Graduation from and accredited University or College with a Bachelor’s Degree in social work or psychology or related field and considerable experience in human services/social services; or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
• Experience working with a Medicaid population is preferred.
• Experience with working with a population that seeks challenges with social determinants of health, including mental health and substance abuse.
