We are seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will play a vital role in providing support and assistance to individuals with various needs. Your primary focus will be on senior care, special education, and working with individuals with developmental disabilities. Duties:
- Conduct assessments to determine the needs of individuals and develop appropriate care plans
- Provide counseling and support services to individuals and their families
- Coordinate with healthcare professionals, community resources, and other service providers to ensure comprehensive care
- Assist with the implementation of group therapy sessions and educational programs
- Collect and analyze data to monitor progress and evaluate the effectiveness of interventions
- Utilize motivational interviewing techniques to empower individuals to make positive changes in their lives
- Advocate for clients' rights and ensure they receive appropriate services and support
- Maintain accurate and confidential documentation in compliance with HIPAA regulations

Experience:
- Bachelor's or Master's degree in Social Work or related field
- Previous experience working with seniors, individuals with developmental disabilities, or special education is highly preferred
- Knowledge of ICD-10 coding system for accurate documentation and billing purposes
- Familiarity with patient care principles and best practices
- Strong understanding of HIPAA regulations and maintaining confidentiality
