Job Overview:
We are seeking a dedicated Registered Nurse to join our Adult Foster Care. As a Registered Nurse, your main responsibility will be to conduct home visits for clients in the community.
This position allows you to schedule your clients according to what works best for you.
To be eligible for this position, you must have a driver's license, a car, and a cell phone, as driving to clients' homes is a requirement,
Responsibilities:
- Conduct client assessments and develop individualized care plans
- Complete PCP and MDS as needed
- Annually and semi annually paperwork
- Client's monthly monthly or bimonthly home visit
- Write progress notes withing 48 hours after visiting client's
Job Types: Permanent, Per diem
Pay: $35.14 - $50.00 per hour
