Summary: Guide patients through complex care, act as advocate and educator, collaborate with healthcare team.

Responsibilities:

• Coordinate, assess, and plan patient care across healthcare journey.
• Assist with finances, resources, and quality improvement.
• Provide nursing care, report updates, and participate in quality initiatives.
• Advocate and educate patients, families, and staff.
• Facilitate care transitions and connect patients with community resources.

Qualifications:

• RN license, BSN preferred within 3 years.
• 3-5 years nursing experience, with acute care and/or case management preferred.

Additional Notes:

• May perform some limited nursing tasks under physician supervision.
• Requires strong communication, empathy, and collaboration skills.
