Adelphoi has a Foster Care Case Manager position available that provides services to clients in Foster homes in Huntingdon and surrounding counties. 

DUTIES AND RESPONSIBILITIES:

To coordinate and implement services with delinquent and dependent youth and their birth family within the context of a foster - kinship family setting; to promote a safe environment in which youth may grow socially, emotionally, intellectually, and physically. Duties include:

• Provide regular in-house visits with youth and foster families in order to implement and monitor placement
• Provides support and assistance to foster parents of delinquent/dependent youth.
• Intervenes and provides immediate assistance in crisis situations, as necessary.
• Develops appropriate individual service plans in order to foster emotional, intellectual and social growth and development of youth
• Documents required information within an Electronic Health Record system (i.e., medical information, daily communications, casework progress notes, etc.) and completes required reports in a timely manner
• Works with foster parents, birth parents and county officials as well as others involved in youth’s case
• Network with internal departments and external agencies.

QUALIFICATIONS: Bachelor’s degree required. Social Work or Psychology degree preferred or 6-12 months experience in field.
