Requirements:

• Valid and active Optometrist license issued by the California State, Board Optometry

• At least 12 consecutive months of performing similar services within the last 3 years

• Must have the required certificates relevant to the assigned specialized area

• BLS certification issued by the American Heart Association (AHA) or the American Red Cross (with AED)

• Must be proficient in the English Language (Speaking/Writing)

Responsibilities:

Under the direction of the Facility CEO/CME or designee, the provider will offer services within the permissible scope of practice and in accordance with Facility policies and procedures. The duties of the Optometrist include but are not limited to the following:

• Ensure that all scheduled optometry services are aligned with the patients' needs, and deliver the services as scheduled, except in unavoidable circumstances.

• Provide examination, diagnosis, treatment, prescription information, and simple repairs, as per Business and Professions Code � 3041 et al.

• Refer patients/youth for consultation, diagnosis, treatment(s), and/or surgery to contract physicians and/or healthcare facilities with approval from the Facility CEO/CME or designee.

• Provide professional staff, allied and supportive paraoptometric personnel necessary for all Optometry services at Vendor's expense.

• Maintain organizational and administrative capabilities to carry out the provider's duties and responsibilities under this Agreement and comply with all applicable statutes and regulations regarding Optometry services at Vendor's expense.

• Maintain continuity of work from start to finish.

• Provide optometry visual services and diagnose and treat specific conditions, such as allergies, infectious diseases, and non-systematic inflammations of the conjunctiva, using diagnostic agents and topical products, in accordance with Chapter 7 of the Business and Professional Code and National Standards.

• Optometry services encompass a comprehensive range of examinations, evaluations, and treatments, including but not limited to:

• Full optometry/optometric evaluation/diagnostic assessment, including preliminary testing, ophthalmoscopy, and refraction.

• Partial examination, including anterior segment exam and glaucoma evaluation.

• Additional services such as recheck and mechanical adjustments & repair at no additional charge.

• Optometry examination and prescription issuance.

• Optometric treatment, therapy, and ocular health examination of diabetic patients/youth.

• Individual treatment plan development and coordination of professional and paraprofessional services, including frame selection, dispensing, and fitting of corrective lenses or contact lenses when medically necessary and approved by the Facility CEO/CME or designee.

• Ongoing evaluation and prognostication.

• Referrals for ophthalmic needs when professionally indicated.

• Emergency ocular services for trauma or systemic disease during clinic hours, with coordination of the treatment plan with staff physicians.

• Followup services to ensure continuity of care.
