About Our Program
Our therapeutic group homes provide trauma-informed services and skill building to young men. Residents receive intensive psychiatric treatment for complex trauma within a developmental framework. Up to five boys ranging in age from 14 to 20 live in each home. The treatment plans are carefully tailored to meet the needs of each individual. The programs provide a transitional home for the boys as they move from acute inpatient or out-of-state care to successful community living.
Job Summary:
The Therapeutic Group Home (TGH) Clinician provides individual, group and family therapies for all individuals residing in the Therapeutic Group Home’s.
S/he serves as a member of the TGH’s treatment team, coordinating various components of treatment both internally and externally with other providers. The TGH Clinician is responsible for all treatment aspects of individuals residing in the home. This is a full-time, exempt position that reports directly to the Director of Therapeutic Group Homes (administratively) and the Supervisor of Clinical Services (clinically.)
Duties and Responsibilities:
  • Assists in the intake process by coordinating a preplacement visit and evaluation for appropriateness of individual(s) referred for admission to Adelbrook. The TGH Clinician will work collaboratively with the Director of TGH and the TGH Program Coordinator to ensure all documents needed for admission are obtained.
  • Helps oversee the coordination of intakes, ensuring completion of all admission paperwork, sets up individual’s file for admission. This includes ensuring that all initial client records are input into the Electronic Health Record (EHR) and closed at the time of discharge.
  • Responsible for completing the psychosocial assessment, as well as developing and implementing a comprehensive treatment plan for each assigned individual on assigned caseload.
  • Coordinates implementation of the treatment plan with other team members through weekly treatment team meetings.
  • Works with the TGH Program Coordinator, the co-supervisors, and the Director of TGH to establish and maintain a therapeutic milieu.
  • Coordinates the admission and transitional process for residents/individuals, including maintaining communication with Beacon Health, School Districts, and DCF case managers.
  • Provides individual, group and family therapies for all individuals residing in the home as well as participates in psychiatric rounds for each individual.
  • Facilitates each adolescent’s participation in community groups and school activities.
  • Works to identify professional development needs of staff and appropriate training. Collaborates with the Director of TGH, TGH Program Coordinator and the co-supervisors to address these needs.
  • Collaborates with the Director of TGH, TGH Program Coordinator and the co-supervisors to ensure that all staff have competencies to provide services that meet individual/residents’ needs.
  • Communicates and collaborates with Director of TGH, Supervisor of Clinical Services, TGH Program Coordinator and the co-supervisors, contracted behavioral staff, and team members to develop and maintain a professional work environment that beneficially meets the needs of the residents/individuals.
  • Participates in staff meetings, house meetings, treatment team meetings, ETMs, PIPs, supervision, case presentations, and all other required clinical meetings to enhance the individual’s treatment goals.
  • Serves as the primary liaison between; Ädelbrook, the family/guardian, educational placement, funding source, and all other appointed services, for the individual.
  • Ensures visitation for each assigned individual with family/guardian is supported clinically and communicated to all parties as needed.
  • Maintains and updates records, in a timely fashion. Complete all treatment summaries, progress notes, initial assessments, MTTPRs, and discharge summaries in collaboration with the Director of TGH and the Supervisor of Clinical Services as prescribed by PNMI and ethical standards.
  • Oversees the maintenance of files (EHR and hard files) to ensure that they are maintained in a confidential and orderly manner. This includes preparation for PNMI, DCF and COA licensing visits.
  • Ensures that all DCF contract licensing, PNMI and COA best practices are followed throughout all programs.
  • Coordinates discharge planning in collaboration with all applicable professions including external agencies.
  • Maintain professional competency by participating in workshops, seminars, and all other requirements mandated by one’s licensure or discipline.
  • On-call coverage to support the Therapeutic Group Homes during off hours and/or as requested.
  • Displays exemplary role modeling on the job and when in contact with the individuals being served.
  • Perform all other duties as assigned by the Director of TGH and the Supervisor of Clinical Services.
Education, Experience, and other Qualifications:
A Master’s degree in Marriage and Family Therapy, Social Work or related field from an accredited institution is required; Clinical license preferred. All MSW’s must possess their LMSW to be eligible for hire. S/he must have a demonstrated ability to collaborate with other agency employees, community service organizations, the Department of Children and Families, and the Department of Developmental Services. The ability to work with persons of diverse ethnic, economic, educational and religious backgrounds is required as are excellent verbal and written communication.
