- 1 hour of stroke education to be completed by the end of orientation and then annually thereafter.
- Home/taxable address.
- Skills checklist (must have been completed within 1 year of submission date).
- License verification.
- Copy of BLS (AHA) or ARC.
- References - highly preferred.
- Iowa license (Compact Allowed).
- BLS (AHA).
- NIHSS.
