POSITION: Occupational Therapist
Location: St. Stephens, WY

Qualification:
· Master’s Degree in Occupational Therapy

Experience, Certification, and Licenses
· Number of Years of Directly Related Professional Experience: 5 years of experience,
preferably with Native American students
· CERTIFICATION(S): National Board for Certification in Occupational Therapy (preferred)

· LICENSE(S): State license (License in state where services are to be provided)
· REFERENCES: Must provide 3 references. Reference shall be listed for each to include current and direct contact information with e-mail. Candidate must provide evidence of active unrestricted state license/certifications.

Tasks:

• Review student's medical records, physician referral, child study team referral, parent
• Request and/or school psychologist evaluation recommendation for further testing.
• Perform evaluation procedures to establish performance guidelines.
• Provide formal written comprehensive test results of student assessment 30 days from the evaluation date.
• Plan, prepare, and establish appropriate written Individual Education Plan (IEP) goals and objectives.
• Attend the student's scheduled IEP meeting.
• Administer and implement physical therapy/occupational therapy programs.
• Re-assess and revise the program, as necessary, as the status of students changes.
• Instruct and teach students, families, and teachers as indicated on IEP.
• Make referrals to other health professionals and integrate physical/occupational therapy programs with other aspects of health care.
• Confer with physicians and other health practitioners to obtain information and make recommendations and revisions to the treatment program.
• Supervise and train education technicians and/or certified teachers in occupational/physical therapy assistance to be carried out when the occupational/physical therapist is not at the school.
• Provide weekly on-site services during the school year as warranted by student needs;
• Provide a progress report for individual students to coincide with the school's grading periods.

Services shall be provided 2 times a week for 1.5 hours a day for 40 weeks throughout the school year and coordinated with the school. Service hours may increase or decrease based on student evaluations and will be scheduled and coordinated on an ongoing basis.
