All functions are performed in accordance with the Plan of Care and the company’s policies and procedures. Participates in the development of the Plan of Care.Prepares social histories to augment existing service, or as a guide in determining or changing level of service, taking into consideration:Family relationshipThe meaning of illness to the client and his and her familyThe cultural attitudes in relationship to illnessThe meaning of home to the clientThe ability of the client and his family to adjust to the client at homeThe financial impact of illness upon the familyCounsels, supports, mobilizes and works with clients and families to ensure basic human needs are met. Assesses compliance with the medical treatment plan.Refers clients and families to community agencies when appropriate.Assesses, when appropriate, a family's financial situation, taking into consideration client's prognosis and medical needs.Prepares clinical and progress notes. Reports to physician and the Case Manager the client's response to treatment or change in condition.Maintains appropriate records including frequency of visits and client's response.Acts as a consultant and interprets social resources to staff and health services to social agencies. Communicates effectively with all members of the interdisciplinary team.Participates in discharge planning and inservice programs.Maintains absolute confidentiality of all information pertaining to clients, families and employees.Maintains a safe client environment and identifies and reports to the office any suspected vulnerable client abuse, neglect, or financial exploitation.Performs other related duties and responsibilities as assigned.PHYSICAL/ENVIRONMENTAL DEMANDSLight to Medium. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather. Exerting 20 to 50 pounds of force occasionally, 10 to 25 pound of force frequently, and greater than negligible up to 10 pounds of force constantly to move objects. Physical demand requirements are in excess of those for light work. QUALIFICATIONSMasters Degree from a school of Social Work accredited by the Council on Social Work Education.Currently licensed as a LMSW in the state of Minnesota. The license must not have been revoked, suspended, and without limitations or restrictions.Have at least one (1) year of social work experience in a health care setting or equivalent experience. Home care experience preferred.Strong interpersonal communication and teaching skills.Strong organizational skills and the ability to work independently.Knowledge of public health administration and demonstrated ability to work within a framework of social work consultation and supervision.Knowledgeable of resources in the community to refer clients needing assistance.Access to a dependable vehicle or public transportation in order to travel to multiple company business stops a day.A valid driver’s license and proof of car insurance when using a personal vehicle for company business.Proof of negative mantoux or documentation of negative chest x-ray.Current CPR certification recommended.Have U.S. Citizenship or evidence of valid Alien Work Permit.Discloses any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of such crimes will not be hired.Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
