Action Home Care South Florida is Hiring a PT/PTA Near Jupiter, FL
- Provide therapeutic treatments and evaluation in patient’s home.
- Advising and consulting with the family and other agency personnel as needed.
- Preparing clinical progress notes.
- Evaluating outcomes of care.
- Discharge planning.
- Developing and revising patient care plan as needed to ensure optimal patient outcome in consultation with the physician and other care-team members.
- Assess functional needs of patients, including provision of direct care, establishing goals, and evaluation of progress
- Complete initial and ongoing OASIS assessments at appropriate time points.
- Supervise PTA and HHA as needed.
- Design and implement patient care plans based on assessment results
- Consult with specialist regarding patient care and progress
- Conduct reassessment of patients needs and care plan adjustments as needed to ensure optimal patient outcome
- Educate patient/family regarding functional status and safety needs
- Document evaluations and treatment goals/plans, including regular updating of documentation, as well as obtaining required prescriptions and related signatures
- Maintain established facility and departmental policies and procedures, objectives, quality and improvement programs
- Participate in educational programs, current literature, in-service meetings, and workshops
- Provide direction and leadership to support staff, students and new graduates
- Perform other related duties as assigned
- Graduation from an accredited physical therapy program and current Florida licensure.
- Skill in the use of computers, preferably in a PC, Windows based operating environment
- Familiar with basic CPR rescuer skills
- Excellent interpersonal and oral/written communication skills
- Strong organizational skills
- Ability to manage multiple projects, tasks and priorities to achieve desired goals
- Maintain confidentiality and integrity of position
