Hiring for a

Registered Nurse with Case Management/ Disease Management Experience

Requirements:

• Must have a valid Registered Nurse license in the State of Florida or Compact License
• Must have at least 3 years of nursing experience in Remote Case Management and Disease Management.
• Strong team-oriented mindset with a passion for patient care,
• Strong computer skills

Responsibilities -including but not limited to:

• Telephonic case management for patients - which includes patient education and managing continuous care.
• Accomplishes patient care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress.
• Responsible for complying with policies and procedures under nursing laws and regulations
• Case management/ telephonic and remote experience is REQUIRED
