Under the direction of the Care Management Director, this position is responsible for facilitating improvement in the overall quality and completeness of documentation within the medical record.
Education, Knowledge, Skills and Abilities
Strong organizational and prioritization skills and demonstrated leadership skills. Excellent communication skills adaptable to individual levels of understanding. Critical thinking, problem solving and deductive reasoning skills. Dependable, self-directed and pleasant. Able to participate collaboratively with all members of the care team. Adaptable and flexible to meet customer needs. Ability to learn and develop the skills necessary to perform the CDI (Clinical for Documentation Integrity) program. Basic computer skills. Familiarity with Windows-based software programs.
Required Length and Type of Experience
Minimum of three to five years of clinical nursing experience required. Minimum of two years experience in Utilization, Quality or Case Management preferred. Knowledge of acute care regulatory/accreditation requirements preferred. Discharge planning and/or Quality Improvement experience preferred.
Required Licensure or Certification
State Licensed Registered Nurse. BSN preferred. CCM preferred.
Required Physical and Environmental Demands
Requires direct patient contact. Require extensive walking, telephone use, bending, reaching, and lifting items weighing twenty pounds or less. Must be able to work well under pressure and maintain professional demeanor under adverse conditions. Extensive reading, writing, use of pagers, fax machine, computers, and copiers.
