Licensed Practical Nurse (LPN)

About Us: Join ACI Federal's dynamic healthcare team! We are currently seeking a skilled and compassionate Licensed Practical Nurse (LPN) to contribute to the delivery of exceptional long-term care.

Position Overview: As an LPN, you will play a vital role in providing high-quality care. Your responsibilities will include:

  • Assisting new residents in adjusting to the unit, philosophy, and goals.
  • Reporting any accidents or changes in the resident's condition to the Charge Nurse and documenting on progress notes in the chart.
  • Collaborating with the interdisciplinary team to prepare, implement, and evaluate care plans.
  • Discussing clinical issues regarding resident care with the Charge Nurse.
  • Observing, recording, and reporting progress or changes in the resident's condition.
  • Updating care plans as needed.
  • Administering and dispensing medications (excluding IV) to residents, observing for side effects and effectiveness.
  • Securing and accounting for controlled drugs.
  • Carrying out therapeutic treatments.
  • Maintaining nursing care and administrative records and reports.
  • Attending staff meetings and participating in in-service programs.

Minimum Qualifications:

  • Completion of a course of study approved by the New York State Education Department as qualifying for Licensed Practical Nurse.
  • Licensure to practice as a Licensed Practical Nurse in New York State.
  • COVID Vaccination required.
