Position type:

  • Full-time, hourly
  • Weekend on-call run Friday through Sunday.
  • This is full-time position that consists of 37.5 hours during the weekend. Billable time is acquired by handling phone calls and various situations within the homes. If, at the end of the weekend, the billable time does not add up to 37.5 hours, staff would have to fill shifts in order to meet those full-time hours.

Must have personal vehicle to get to each site. Agency vehicle used any time a person we support must be transported.

Position Description:

Serves as the staff member with cellular phone responsibilities during designated time frames for staff support, assistance and guidance. Ensures compliance with Pennsylvania Chapter 6400 Regulations and the Pennsylvania Department of Human Services standards. Reports to the Assistant Director.

Job Responsibilities - Highlights:

  • Supports the achievement of the individual's personal outcomes.
  • Implements the Residential Supports Department Policies and procedures relative to Community Homes.
  • Develops and implements strategies and plans for program improvement. Provides for adequate staffing levels at each home and authorizes staff assignments and reassignments. All done with prior approval.
  • Communicates and resolves issues and/or concerns with the Health Care Manager. Informs the Community Home Specialist of any health, safety and welfare issues as well as individual, staff and home status.
  • Responds to staff by phone or on-site in the event of a crisis situation. Ensures staff adheres to the appropriate procedures and that the appropriate people, both professionals, and family have been notified.
  • Participates on various committees and work groups as assigned.
  • Maintains an appropriate level of communication with family or guardians, day supports, advocates or other support service personnel as directed.
  • Maintains awareness of new developments and current trends in the area (s) of expertise. Receives 24 hours of training relevant to Human Services.
  • Acts as a Designated Agency Person per the Medication Administration requirements.
  • Must obtain CPR certification within six months of the start date.
  • As needed, assigns work and arranges work schedules for employees. Ensures that procedures are followed.
  • Provides guidance, direction, and support for staff. Identifies areas of concern and/or reports to supervisor any observed problems in performance.

In addition to paying for federal and state clearances, Achieva offers 24 hours of training, annually, relevant to Human Services. In addition, we have implemented Medication Administration training, which would need to be completed successfully. This training assists our DSPs in ensuring all prescribed medication is dispensed and documented per P&P in partnership with the individual.

Minimum requirements:

  • Equivalent of an Associates degree and two years of direct support experience.
  • Must meet Act 33 and 34 requirements and FBI background check.
  • Valid Pa drivers license and properly insured automobile.
  • Must obtain First Aid/CPR certification within six months of start date.
