Job description

Accord Hospice is seeking a self-motivated Medical Social Worker in the White Mountains area. Become part of our care team that assists those in need while still being able to maintain that home and work balance, pay is negotiable. Very Flexible Schedule.

JOB SUMMARY

To deliver varied social work services to patients and their families in whatever place they call home. To provide initial emotional, spiritual, psychosocial assessments and ongoing bereavement services and community education and outreach referrals.

QUALIFICATIONS/PROFESSIONAL REQUIREMENTS:

Experience as a hospice social worker is preferred, but will train the right person. Have an Bachelor’s degree or Master's degree of social work.

DUTIES AND RESPONSIBILITIES:

  • Performs the initial psychosocial evaluation and re-evaluation visits to identify the patient's needs for hospice care, treatment and/or services and progression toward goals.
  • Evaluates and regularly re-evaluates the patient's needs for Medical Social Work care, treatment and/or services and clinical status.
  • Initiates the plan of care and assist with the hospice admission process
  • Evaluates the patient's response to the hospice plan of care in a timely manner as required by Agency policy and applicable laws, regulations and standards.
  • Ensures coordination of hospice patient care, treatment and/or services
  • Reviews and updates the hospice plan of care as needed, but no less frequently than every 14 days during IDG.
  • Provides and documents all Medical Social Work care and services in accordance with the hospice plan of care, and informs other team members, including the physician, of any changes in the patient's condition and/or needs
  • Instructs patients and their families/caregivers in a manner and format they can easily understand.
  • Instructs and supervises other personnel and hospice aides as appropriate and in accordance with applicable laws, regulations, standards, and Agency policies
  • Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed IDG care plan.
  • Conducts ongoing reassessments of patient/family needs and counseling as required.·
  • Provides short term crisis intervention and individual or family counseling when indicated.
  • Provides services, under the direction of a physician (who approves plan of care).
  • Participates as a member of the Bereavement Team as assigned.
  • Observes, assesses and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.
  • Assumes the active role of advocate for the patient/family unit.
  • Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
  • Documents comprehensive psychosocial, emotional and spiritual assessment clearly and concisely in a timely manner.
  • All patient/family visits, telephone contacts and referral actions are recorded in the patient record per policy.
  • Provides ongoing counseling related to issues of death and dying to the patient and family as needed.
  • Attends staff meetings, IDG and other meetings as assigned and appropriate.
  • Participates in the orientation program as assigned.
  • Adheres to all Hospice policies.
  • Assumes responsibility for own personal and professional development and maintenance of skills in social work.
  • Exhibits Hospice philosophy in all job-related roles.
  • Other duties as assigned by Director of Nursing .

MENTAL REQUIREMENTS:

  • Must be able to work independently make judgments based on assessments and date available and act accordingly Must be flexible, innovative and possess good interpersonal skills .
  • Must have a current valid driver's license, auto liability insurance and reliable transportation.

Pay: From $68,000.00 per year
