Hours: M-F: 8:30am-4:30pm Flexible depending on participant needs

Salary range: $65,000 - $75,000

Productivity bonus twice yearly

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings. 

About the Role 

We are looking for a proactive employee with minimal supervision, uses professional judgement and knowledge of current and evidence based Social Work to provide Social Work services to children and adults with a range of developmental, physical and/or health related disabilities. Responsibilities include: evaluation, development of Plan of Care (POC), provision of individual, group and family counseling, and completion of documentation including evaluations and treatment note. Supervise LMSW and interns/students.

Responsibilities include:

• Complete Social Work initial evaluations and revaluations for individuals on clinician’s caseload. 
• Using evidence based techniques, provide assessment and individual, group and family counseling for participants and their family members. 
• Complete all program documentation in a timely manner and per Agency standards. Documentation includes but not limited to: treatment plans, treatment notes, initial evaluations, billing forms, and discharge summaries.
• Provide information and resources regarding diagnoses, disability and treatment. 
• Provide initial and ongoing training to LMSW and interns in conjunction with Clinic Director.
• Maintain appropriate contact with assigned participants and their families, guardians and care managers.

Education & Experience 

Current NYS LCSW license required. 1 year working with children and adults with developmental disabilities and/or a mental health diagnosis preferred.

Conditions of Employment

Must possess valid New York State driver’s license and transportation necessary to meet program needs. Must be able to lift 75 pounds. Must be willing to work late afternoon/early evening hours based on therapy needs of assigned caseload. Successful completion of required orientation and annual training. Position requires good verbal and written communication skills. Must be able to demonstrate teamwork and professionalism. Basic word processing and e-mail skills require. May be requested to complete an evaluation or training in an individual’s home.
