Access TLC Health Care is Hiring a Social Worker/Case Manager Near San Fernando, CA

Access TLC is an HCBA waiver agency, contracted with the Department of Healthcare Services (DHCS). The Home and Community Based Alternatives Waiver program provides care management services to persons at risk for nursing home or institutional placement. The care management services are provided by a multidisciplinary care team comprised of a nurse and a social worker. The care management team coordinates Waiver and State Plan services (e.g., medical, home health, In-Home Supportive Services, etc.), and arranges for other available long-term services and support available in the local community. Care management and Waiver services are provided in the Participant’s community-based residence. This residence can be privately owned, secured through a tenant lease arrangement, or the residence of a Participant’s family member. Service areas include certain jurisdiction in Los Angeles, Santa Barbara, and Ventura Counties.

Key duties and responsibilities include, but are not limited to the following:

· Work collaboratively with Registered Nurse, staff, and management to be compliant with DHCS rules and regulations.

· Work with the HCBA participants, their legal representatives, circles of support, and primary care physicians to educate, counsel, and advocate to and on behalf of participants.

· Document assessments, case notes, care coordination, and utilization management within the MedCompass system, a health management software

· Develop goals associated with the participant’s assessed needs and circumstances to provide resources.

· Develop a Plan of Treatment (POT) with the RN to identify areas of concern.

· Assist with care coordination, service providers and community resources to help assure the timely, effective, and efficient mobilization and allocation of services.

· Monthly case management calls to maintaining ongoing contact with participants and to monitor for changes in health, mood, social integration, functionality, and overall well-being.

· Complete APS reports and related reports to the state based on mandated reporting criteria and follow through with addressing issues and concerns related to changes in condition due to abuse/neglect.

REQUIRED QUALIFICATIONS

· Bachelor’s degree in any of the following :

o Social Work,

o Psychology,

o Counseling,

o Rehabilitation,

o Gerontology, and

o Sociology

· Experience with, and understanding of the medical and psychosocial problems of adults, functionally impaired children, adults with disabilities and older adults,

· Demonstrated case management skills and ability to prioritize tasks,

· Excellent detail orientation and problem-solving skills,

· Excellent Computer literacy (Excel, Outlook)

DESIRED QUALIFICATIONS

· Experience working with individuals with various medical conditions, mental and/or behavioral health diagnoses and substance abuse disorders.

· Exceptional interpersonal, oral, and written communication skills

· Multilingual preferred

Pay: Starting $22 an hour
