Responsibilities:

- Conduct comprehensive evaluations for clients/patients to assess their home care needs.

- Collaborate with healthcare professionals and family members to develop individualized care plans.

- Provide recommendations and referrals for appropriate home care services.

- Document assessments, care plans, and recommendations accurately and in a timely manner.

- Communicate effectively with clients/patients, caregivers, and interdisciplinary team members.

Requirements:

- Current and valid Registered Nurse (RN) license in the state of Colorado.
