Job Summary:
The Medical Assistant works with a dedicated Primary Care Provider and supports primary care activities for the PCP panel, including facilitating PCP visits, providing direct patient care, closing quality gaps, and managing the PCP schedule. The Medical Assistant works directly with members to achieve their care plan goals through care coordination and building relationship.
