Nurse Practitioner - Palliative Care & Hospice
Come join our family owned company! Absolute Hospice is presently seeking a Nurse Practitioner to join our Hospice Team.
Education:
Graduate of an accredited school of Advanced Practice Registered Nursing
End of Life Nursing Education Program (ELNEC) to include all 8 modules must be completed within 18 months of employment
Current Advanced Practice Registered Nurse License within the state of Ohio
MSN or Doctoral Degree required
Certification in Hospice and Palliative Nursing desired
Experience:
Minimum of one year experience as an Advanced Practice Registered Nurse, end of life care experience preferred
Core Competencies:
Demonstrated competency in performance of professional nursing functions
Requires excellent oral and written communication skills
Ability to work with dying patients and their families
Possess a sympathetic attitude toward caring for the hospice patient and demonstrate positive communication skills in interacting with other members of the hospice team
Accurately assess the eligibility of patients for hospice service
Other:
Valid driver's license and auto insurance
Summary of Major Functions:
Performs comprehensive nursing assessments
Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
Initiates plan of care as needed
Coordinates as needed, all services provided to patients/families in all settings to ensure quality of care and services as well as appropriateness of care
Provides direct Face to Face visits to referred patients and patients already receiving hospice services; contacts physician for update on patient's eligibility and clinical status
Demonstrates flexibility in scheduling, and readily accepts new admissions and emergency assignments as requested
Maintains knowledge of state and federal regulations and ACHC standards (if applicable)
Particpates in QAPI program
Observes for safety hazards in the patient's and organization's environments and takes appropriate steps to reduce or eliminate them
Maintains continuing education through academic studies, seminars and workshops
Attends and participates in in-services, staff meetings and patient care conferences
Completes all documentation in an accurate and legible manner
Complies with hospice policies and procedures, laws and regulations and standards of practice
