Responsibilities:
- Provide direct and individualized nursing care to patients based on the application of scientific nursing principles
- Perform comprehensive assessments of patients' health status and develop personalized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and document patients' vital signs, symptoms, and changes in condition
- Collaborate with interdisciplinary healthcare team members to ensure quality patient care
- Educate patients and their families on disease management, treatment plans, and preventive care measures
- Assist with diagnostic tests and procedures, such as sonography and catheterization
- Provide health coaching and support to patients to promote healthy lifestyle choices
- Coordinate and provide home care services for patients requiring ongoing medical assistance
Qualifications:
- Valid Registered Nurse (RN) license in the state of practice
- Minimum of 2 years of nursing experience preferred
- Experience in infant care, assisted living, or home care is a plus
- Proficiency in using electronic medical record systems such as Cerner
- Ability to catheterize patients when necessary
- Knowledge of durable medical equipment (DME) usage and maintenance
- Familiarity with conducting sonography procedures for diagnostic purposes
- Strong assessment, critical thinking, and problem-solving skills
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
