Medical Social Worker - Hospice

ABL Hospice, LLC the Hospice agency is seeking an experienced Medical Social Workder with at least 1 year of Hospice experience. By joining our team providing the highest quality experience for patients in the comfort of their own home.

Job Description The role of the social worker within the core interdisciplinary team centers on assessing the psychosocial symptoms of patients and their families. The social worker is responsible for formulating psychosocial outcomes to alleviate identified symptoms and for delivering psychoeducation, supportive counseling, and casework interventions to achieve these outcomes. Additionally, the social worker provides psychosocial education and consultation to the Interdisciplinary Group (IDG).

Job Responsibilities

• Contributes to the preparation, assessment, and execution of the interdisciplinary care plan, ensuring the provision of high-quality psychosocial care to patients and families while maintaining care continuity and comprehensiveness.
• Conducts bio-psycho-social-spiritual assessments of patients and their families, focusing on their psychosocial well-being rather than performing diagnostic assessments.
• Offers individual and family supportive counseling, rather than engaging in psychotherapy.
• Makes appropriate referrals to community resources as necessary.
• Maintains accurate and timely documentation in the patient's medical record.
• Participates in weekly team meetings for care plan reviews.
• Takes part in patient case conferences.
• Provides psychosocial education and consultation to the core IDG.
• Attends psychosocial supervision meetings and other training sessions. May provide supervision to other psychosocial team members, including social work interns, under the guidance of the team manager.
• Can act as the coordinator and communicator for volunteer interventions as outlined in the care plan.
• Provides bereavement care, including organizing bereavement group activities.
• Participates in the psychosocial spiritual on-call rotation/schedule as assigned.

Job Requirements

• Willingness and ability to travel as required by the job.
• Profound knowledge of community resources or a strong desire and ability to acquire this knowledge.
• Familiarity with significant psychosocial issues commonly experienced by terminally ill patients and their families/caregivers, including effective psychosocial interventions to address these issues.
• Capability to collaborate effectively with others as an integral part of an interdisciplinary team.
• Exceptional listening and communication skills.
• Ability to advocate effectively for patients and their families, both within the patient/family unit, within the core IDG, and within community agencies, as needed.
