MUST HAVE HOSPICE EXPERIENCE

ABL Hospice, LLC the Hospice agency is seeking an experienced Licensed Vocational Nurse (LVN) with at least 1 year of Hospice experience. By joining our team providing the highest quality experience for patients in the comfort of their own home.

Responsibilities and Duties for LVN Hospice:

• The Licensed Vocational Nurse (LVN) is accountable for delivering direct patient care under the guidance of a registered nurse. Responsibilities encompass adhering to the care plan, administering treatments, and collaborating with the healthcare team members to contribute to positive patient care outcomes.

• The LVN provides direct patient care in accordance with California regulations.
• Executes current nursing protocols based on a comprehensive assessment and the care plan.
• Ensures precise and punctual documentation of patient services to align with the care plan.
• Evaluates and offers patient and family/caregiver education and information relevant to the diagnosis and self-care plan.
• Engages in the coordination of hospice services, effectively conveying identified needs to the interdisciplinary group, which may include hospice aides, occupational therapists, physical therapists, social workers, speech therapists, dietitians, or clinical supervisors.
• Demonstrates proficient use of equipment and supplies.
• Administers appropriate pain/symptom management and assesses the patient's response to treatments/medications.
• Participates in personal and professional growth and development, as well as engages in activities related to quality assessment and performance improvement teams.
• Performs other duties as assigned by the registered nurse.

  Job Requirements

• Graduation from an accredited practical nurse or vocational nursing program.
• Three years of nursing experience, with a preference for experience in community health/hospice or medical/surgical settings.
• Current licensure as an LVN in California.
• Adherence to accepted professional standards and practices.
• Proficiency in verbal and written communication, as well as strong organizational skills.
• Possession of a valid driver's license, along with an insured automobile that meets state and/or organizational requirements and is in good working condition.
• Maintenance of current CPR certification.
• Understanding of the hospice philosophy and recognition of the needs of terminally ill patients.

Qualifications:

• Current and unrestricted license

• 1 year: Home Health Experience

• Current BLS/CPR certification

• Valid Driver's License
