Responsibilities and Duties for LVN Home Health:

Prepare clinical notes, orders and charting in a timely manner

Collaborate with physicians and agency staff to deliver the best care possible

Work with patient and family in the home to achieve improvement in health goals

Manage patients health care episode with the agency

1 Year experience in Home Health

Reliable transportation

Valid Driver's license

LVN License

Responsibilities:

Work with patient and family one on one to achieve improvement and/or stability.

Prepare clinical notes, orders and charting in a timely manner.

Collaborate with physicians and agency staff to deliver the best care possible.

Qualifications:

Current and unrestricted license

1 year: Home Health Experience

Current BLS/CPR certification

Valid Driver's License
