Starting Salary

The starting pay is $20.00 per hour. To qualify for a higher pay rate, the candidate will have to provide documentation of relevant experience and qualifications.

Job Overview

As a Community Navigator, you'll play a pivotal role in empowering individuals with developmental and/or physical disabilities. This position involves working closely with participants, their support teams, and service providers to develop Person-Centered goals and objectives. You'll facilitate meaningful community engagements, ensuring each participant’s safety and personal growth.

Key Responsibilities

  • Conduct assessments to determine participants' interests and needs.
  • Coordinate with a diverse team to support participants' integration into the community.
  • Advocate for participants, ensuring they have access to necessary services and support.
  • Develop, implement, and monitor community-based programs that align with participants' Person-Centered Plans.
  • Serve as a liaison between participants, their families, and community resources.
  • Lead and support staff in delivering high-quality, person-centered services.

Qualifications

  • 12 semester or equivalent quarter units in Early Childhood Education or related courses that are acceptable to meet this standard.
  • Deep understanding of learning methods and services for adults with disabilities.
  • English and Spanish bilingual.
  • Strong case management skills and experience in community program development.
  • Exceptional leadership, team building, and interpersonal skills.
  • Proficiency in personal computer use and relevant software applications.
  • Ability to work independently, make well-informed decisions, and manage multiple projects.
  • Bachelor's degree or equivalent in a related field preferred.

Work Schedule

Full-Time Position
Monday through Friday, 40hrs per week
