Duties and Requirements:

• Implement Plan of Care (POC) with client or client and parent/guardian.
• Attend staff meetings/supervision and training per supervisor’s requirements.
• Maintain contact with referring agency every week to discuss updates, progress, concerns, or provide feedback.
• Adhere to all agency policies and procedures and complete other tasks deemed necessary for the successful operation of services.
• Complete all required documentation which includes, but is not limited to, tracking of the POP and related paperwork.
• Must complete a TB screening within 30 days.
• Must complete and maintain HIPAA Certification within 90 days of starting.
• Maintain CPR and First Aid Certification at all times.
• Must be found eligible via a background check with the respective state, FBI, and Child Protective Services or Social Services.
• Must comply with Medicaid and Licensure required training.

Job Types: Full-time, Part-time

Pay: $23.00 - $27.00 per hour
