Hospice RN Case Manager Job Details:
Responsible for providing care and services for a group of patients, visiting patients wherever they call home which could include private homes, Independent Living Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work as an RN Case Manager.
Hospice RN Case Manager Qualifications:
- 2 years of Hospice experience.
- Case Management experience is preferred but not required.
- IA or Multistate RN License in good standing.
- Active BLS.
- Clean background check upon hire.
