Seeking an MDS Assessor to beresponsible for coordinating assessments and completing MDS forms for allresidents in accordance with State and Federal regulations and facilitypolicies. This role involves obtaining necessary information, preparingschedules, inputting data, and ensuring accurate and timely transmission ofMDS/PRI files to the Department of Health (DOH). Additionally, the MDS Assessorcollaborates with the Quality Improvement Department and performs relatedduties.

Compensation: $110,000 – 122,000

Qualifications:

· Bachelor's Degree in Nursing.

· Minimum five (5) years of experience in clinicalnursing.

· Two (2) years of experience in performing MDScompletion.

· Licensed Registered Nurse with NYS.
