Sign-On Bonus $20,000! Seeking a passionate and dedicated Family or InternalMedicine Physician to provide evidence-based primary care to our patients inalignment with our mission, vision, and values. The ideal candidate will have3 years of primary care experience, be committed to outcomes-based medicine,and be data-driven in their approach to population health. You will besupported by an entire care team, including a health coach, to deliver wholeperson care that is often outsourced to specialists in today's broken system.

Compensation: $275,000.00 - $300,000.00

Education and Experience:

• MD or DO degree
• Completion of a Family Medicine or Internal Medicine Residency, Family Medicine preferred
• Current Board Certification by ABMS (ABFM/ABIM) or AOA
• Current, valid, and unrestricted medical license and valid DEA number
• 3 years of primary care experience
• Experience working in EMR systems, NextGen preferred
• Experience in coding and documentation, including Medicare Advantage and HCC

Responsibilities:

• Provide evidence-based primary care to 10 to 15patients per day for 30 to 60 minute appointments
• Develop effective biopsychosocial patient careplans in partnership with the patient and care team
• Ensure comprehensive care in alignment with the medicalhome model, including collection of comprehensive health history, performingappropriate physical examinations, conducting diagnostic testing, andprescribing medications
• Assess patient population needs and work withteam members to drive participation in annual whole health evaluations, healthcoaching, and other health promotion activities
• Provide clinical leadership, includingparticipation in team meetings, clear proactive communication, regularprioritization of key efforts, process improvement, and actively supportingchange management
• Ensure that all customer contractual needs andexpectations are met in partnership with your care team
• Rotate with other providers in the region toensure coverage after-hours
