• The Hospice RN is an experienced registered professional nurse who initiates and coordinates the hospice care plan, provides skilled and palliative care and coordinates the services provided to his/her patients.
• Develops Plan of Care for assigned patients ensuring all services needed are identified and met
• Works cooperatively with members of the interdisciplinary Hospice team.
• Provides follow-up visits and updates the plan of care.
• Performs prescribed medical treatments under physicians' orders, including pain management and symptom control, conducts assessments and evaluations.
• Counsels and educates patients and their families on treatment plans
• Reads and interprets patient information, making treatment decisions when appropriate
• Consults with supervisors and doctors to decide on the best treatment plan for patients

• Experience in hospice care and case management
• Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
• Effective communication skills, including active listening, writing, speaking, and reading comprehension.
• Good collaboration and teamwork skills.
• Fast and adaptive problem-solving abilities.
• Keen attention to detail.
• In-depth understanding of patient care policies and regulations.

• Current CA RN license
• BLS through the American Heart Association
• ACLS through the American Heart Association
