Job Description:- The Hospice RN is an experienced registered professional nurse who initiates and coordinates the hospice care plan, provides skilled and palliative care and coordinates the services provided to his/her patients.
- Develops Plan of Care for assigned patients ensuring all services needed are identified and met
- Works cooperatively with members of the interdisciplinary Hospice team.
- Provides follow-up visits and updates the plan of care.
- Performs prescribed medical treatments under physicians' orders, including pain management and symptom control, conducts assessments and evaluations.
- Counsels and educates patients and their families on treatment plans
- Reads and interprets patient information, making treatment decisions when appropriate
- Consults with supervisors and doctors to decide on the best treatment plan for patients
- Experience in hospice care and case management
- Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
- Effective communication skills, including active listening, writing, speaking, and reading comprehension.
- Good collaboration and teamwork skills.
- Fast and adaptive problem-solving abilities.
- Keen attention to detail.
- In-depth understanding of patient care policies and regulations.
