A Plus Home Health Services LLC is Hiring a Home Health RN Case Manager (Per Diem) Near Mesa, AZ
A-Plus Home Health Services located in Tempe, AZ. is currently seeking Registered Nurses throughout the East Valley area to facilitate home health care visits PRN/Part Time/Per Diem. This is a great position for those who want to make some extra money on the side or want to test out a job before committing full time.
Responsibilities:
- Follow physician orders and provide skilled care to patients
- Diagnose and establish patient treatment plan of care
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about medication management and disease process
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Coordinate care with all disciplines involved on the case
Qualifications:
- Previous experience in home health, home hospice or other skilled settings
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills\
- GREAT PAY and Flexible hours!
- Bilingual a plus!
- Must have valid Arizona License, vehicle, insurance, and CPR card
