Organizational Relationship: Reports to the designated Supervisor

**Responsibilities**:

• Provide compassionate and personalized care, including assistance with personal care, patient care, medication management, and companionship.
• Foster a warm and supportive environment, promoting the well-being and independence of our clients.
• Support clients' mobility and transportation needs, enabling them to remain engaged and active in their communities.
• Build meaningful relationships with clients and their families, offering emotional support and companionship.
• Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each client.

• Excellent communication and interpersonal skills, with a caring and empathetic approach to caregiving.
• Knowledge and understanding of elderly care, senior care, and patient care principles.
• Reliability, responsibility, and a strong work ethic.
• Flexibility and adaptability to cater to the individual needs and preferences of each client.

Education, Experience and Other Requirements:

• Preferred, previous experience as a caregiver or a genuine passion for providing compassionate care.
• Minimum Education: High school diploma or GED
• Driver’s License
• Provide copy of current TB results or Chest X-Ray
• Due to the nature of the work performed this position works with a vulnerable population, we do require a criminal background check be completed along with a Colorado Adult Protective Services check
• Three Professional references

Functional Abilities:

• Is able to lift, turn and transfer Consumers weighting up to 150 pounds.
• Is able to carry bundles upstairs weighing up to 10 pounds.
• Must be able to stoop and bend.
• Must be able to travel to prospective Consumer’s place of residence.
