Job Description: Behavioral Health Specialists (Case Manager)

JOB SUMMARY: The Behavior Health Specialists/Case Manager is responsible for providing one-on-one case management support and facilitating small groups for organization participants. Case Manager will coordinate individual academic & behavioral supports for students. Case Manager will collaborate with the Behavioral Health Director, Academic Director/Team and School Staff & Faculty to ensure student success (attend IEPs,

Parent-Teacher Conferences, and other student support meetings).

*ORGANIZATIONAL RELATIONSHIPS

Reports To: Behavioral Health Director/Academic Director/Supervisors ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to: DIRECT SERVICE (70%)

At the direction of the Behavioral Health Program Director, Supervisors and School Administrators, case manager will receive referrals to meet on an as needed basis with students.

Provide each client's parent/guardians in the A.B.L.E. program with a consent form/intake packet that needs to be thoroughly completed and signed.

Manages a caseload of up to 25 clients.

Organize student caseload into Tier 1, Tier 2, Tier 3 categories.

Builds a positive relationship with youth based on youth development practices.

Provides one-on-one case management with students/clients on a daily to weekly basis (minimum). Ensure home visits are being conducted at least twice per month per client. Utilize one day to have a late start time in the building to reach students at home.

Creates small peer-to-peer tutoring and mentoring groups. Upper grade groups focuses on graduation requirements and college requirements (applications, SAT prep, letters of recommendation, etc).

Facilitate small social skills groups to promote Social Emotional Learning (How to greet others, eye contact, start a conversation, respond to others, share and take turns, ask for help, shake hands, job interview etiquette).

Offer at least one academically focused group tutoring session in the school library/setting or public library once per week.

Develop an individual success plan for each youth based on mutually agreed upon goals, timelines, and support.

Connects youth with appropriate academic supports; academic HUB, tutoring, academic counselors, extra credit opportunities.

Delivers academic information (school information, graduation requirements, college and career prep) focused on concrete outcomes.

Follows-up with referral service providers to ensure access of additional resources.

Provides feedback to School Team Leads, Supervisors and Behavioral Health Director to communicate goals, strategies, and promising practices.

Facilitates and/or participates in restorative circles for youth on caseload on an as needed basis.

Communicates with clients’ caretakers on an as needed basis to rely strengths, behaviors, and resources.

Coordinates with school-day supports such as IEP and IRT Teams, and faculty to ensure communication of goals, strategies, and progress.

Attends appropriate youth enrichment field trips and/or workshops & activities.

Maintains and manages accurate records and data of student activity and submits regular qualitative and quantitative progress reports (ICANotes, Academic binder, Google Doc notes, grade cards, progress reports, etc.).

Understands and follows thru with mandated reporting procedures. GENERAL ADMINISTRATION (25%)

Works closely and collaboratively with School Staff (School-based and Outpatient). Assist students in completing work outside the classroom

Completes administrative tasks such as case management logs, timesheets, and check requests in accordance with organization policies.

Monitor the needs of persons served mental illness or substance abuse and physical symptoms as well as their response to treatment. Seeks direction and feedback from his/her supervisor and uses it as motivation to produce better results for the team.

Completes all tasks thoroughly and on time.

Attends and participates in regular staff and supervision meetings as needed.

Assumes additional responsibilities as assigned by the supervisor. COMMUNITY PARTNERING (5%)

Maintains communication bridges with other youth serving organizations that provide services for youth and families. Follows all A.B.L.E. policies and guidelines.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

Experience: Experience working with youth. Knowledge requirements, knowledge of key educational system. Experience working with a diverse community of youth.

Education/Certification:

High school diploma and three years of relevant experience, CDCA

Preferred: Bachelor’s Degree

Leadership: Natural and proven leader who inspires youth to reach their full potential.

Communication: Poised communicator, motivator, able to write and speak persuasively, to build, empower, and coach youth.

Team work: Excellent collaborator with groups of people in order to achieve a goal. Able to cooperate, using individual skills and providing constructive feedback, despite any personal conflict between individuals.

Problem-solving: Proven solution-finder contributing to productive outcomes; ability to eliminate or reduce opportunities for error through planning and prevention; and ability to produce quick, effective solutions when necessary.

Planning and organizing: Strong planning and organizational skills for long-term and short-term plans.

Technology: Skilled in technology necessary to effectively execute tasks. Timeliness: Punctual administrator of case management logs.

PHYSICAL JOB DESCRIPTION

Typical Working Conditions: School classroom-type environment. Equipment Used: Computer, telephone, printer, fax.

Essential Physical Tasks: * This job requires frequent lifting, pushing and pulling under 10 lbs and occasionally up to 20 lbs.

Because of the use of the computer, this job frequently uses the keyboard which requires repetitive motion and dexterity. In the course of the job, frequent walking sometimes over uneven and slippery surfaces, and sitting for periods of time.

Job Type: Full-time

Pay: $37,000.00 - $41,000.00 per year
