Position Summary:

This position is responsible delivering appropriate care and therapeutic services, monitor and reporting the client’s progress status through appropriate channels according to the best ethical and professional standards. The specific work of this position will be commensurate with special skills, training, and professional degree as a clinician. This position operates under minimal supervision and must exercise tact, judgment and confidentiality in contact with the public, clients and families, as well as fellow associates of the Center. Work is very broad in scope and requires applications of complex technical principles and practices to profession as well as a high level of professionalism.

Qualifications


Requirements:

Education:

  • Master’s degree in Social Work or related field preferred.

Background Checks:

  • Candidates must be willing to submit to comprehensive background checks, including but not limited to criminal and child protective services checks.

Experience:

  • Knowledge and experience of appropriate therapeutic practices as required by Center services assignment.

Essential Duties and Responsibilities:

  • Complete client assessments that include intake, evaluating a client’s ability to meet the admission criteria for that particular department, identify presenting problem(s), history, medical information, family involvement, psychosocial/psychosexual information, and other pertinent information to form an appropriate clinical impression including substance use/abuse and abuse/neglect. This also includes CANS/ANSA, Comprehensive Assessment (including substance uses), Brief Client Activities Assessment, Safety Plan, ACE, and PHQ9/GAD7
  • Complete an intake (biopsychosocial assessment) within 24 hours of client admission to ACU and 24 hours of client admission to CSU.
  • Conducts security screening and inventories property of individuals upon admission to the facility. Provides orientation to the Observation Unit and Short-term Inpatient Unit. Educates patients on their rights under ADHS regulations, grievance process, provides expectations, and explains rules and regulations.
  • Intervenes with escalated individuals using therapeutic techniques for de-escalation and performs Advanced Physical Skills when needed to ensure the individual’s safety and the safety of others.
  • Completes 1-to-1 on a patient Q15 when required.
  • Complete a treatment plan (ISP) which accurately reflects the client’s needs and family and/or natural support/involvement to aid in their ability to meet discharge criteria with goals and objectives that are time limited and behaviorally stated.
  • Complete and document authorized screening tools for each client
  • Provide individual and group interventions, including family, to be completed while on the ACU/CSU and ensure clients are receiving appropriate care based on their individual needs.
  • Provide evidence-based treatment through Illness Management and Recovery (IMR).
  • Maintain regular contact with ACU/CSU client’s current outpatient clinician to ensure continuity of care.
  • Maintain client contact schedule and document client contacts using appropriate ACU/CSU codes in EHR.
  • Document timely and accurate information, in accordance with Center policies which reflects client progress based on the appropriate therapeutic modality. Therapeutic services will be time limited and specific to the presenting problem(s) of each assigned consumer.
  • Provide progress reports to referral sources as needed for each individual client.
  • Periodically communicate with the referring agencies, at least monthly, to provide progress updates and obtain relevant collateral information from the referral source and other external treatment team members.
  • Report client progress to the treatment team during rounds.
  • Participate in treatment team meetings with others involved in the client treatment plan (internally and externally).
  • Provide education, both to the client, and the client’s significant other/family about the client’s illness.
  • Provide information to clients and their families about available community resources.
  • Provide individual, group, family therapeutic services, and case management as needed to clients based on the assessed need.
  • Ensure that the setting in the Acute Care Unit and Crisis Stabilization Unit is therapeutic and that evidence-based practices are utilized for positive client outcomes.
  • Provide emergency services including risk assessment, preliminary diagnoses, and safety planning, as well as coordination with internal and external entities staffing appropriately with Licensed Independent Practitioners (LIP) as needed.
  • Attend and testify, if needed, at client court hearings.
  • Coordinate and complete necessary court documentation for inpatient commitments such as EDO, IDO, etc.
  • Complete all assessments and paperwork required for payer sources, including presumptive eligibility as appropriate.
  • Complete required paperwork for client’s long term placement in a timely manner.
  • Maintains confidentiality of client records
  • Maintain positive working relations and rapport with the communities served.
  • Complete discharge in EHR and review discharge planning with client
  • Schedule all follow up appointments for each client, rescheduling as needed.
  • Provide referral to dentist and primary care physician for each client as well as all other recommended referrals.
  • Complete and document family contact for each client within 24 hours of admission.
  • Ensure all ISPs are completed with the client’s own goals and ISP’s are signed prior to client’s discharge from ACU and/or CSU (if applicable).
  • Provide a clinical one on one with each client, at minimum, one time a day and document
  • Update COP and SSACT Spreadsheet accordingly and send to CQI and all pertinent parties.
  • Send daily end of shift email to keep staff informed of daily progress for each client.
  • When determined by Supervisor, CDON or Medical Director that census and acuity is low, this position may be reassigned to similarly situated 24 hour or high acuity alternative programs to support agency consumer care (Group Home etc.). Become a certified trainer in CPI/APS and assist in ongoing training to maintain certification and facilitate crisis drills for group homes and ACU/CSU
