Responsibilities:
- Conduct outreach activities to identify and engage individuals in need of assistance
- Educate individuals on available resources, including addiction counseling, public health programs, and housing options
- Collaborate with community organizations and healthcare providers to coordinate services for clients
- Collect data and maintain accurate records of outreach activities and client interactions
- Utilize motivational interviewing techniques to encourage individuals to seek support
- Assist with referrals and enrollment in appropriate healthcare or social service agencies
- Attend case conference meetings with other providers

Qualifications:
- Previous experience in outreach work or related field is a plus
- Knowledge of local social services is beneficial
- Strong communication skills, both written and verbal
- Ability to build rapport with diverse populations
- Excellent organizational skills and attention to detail
- Proficient in data collection and record keeping
