Care Coordinator

Duties:

- Coordinate and schedule Home Health Aides with Cases
- Manage patient records and ensure accuracy of information
- Communicate with patients to provide information about their appointments and answer any questions or concerns

- Assist with insurance verification and claims processing

- Excellent communication skills, both verbal and written
