365 Health Services is Hiring a Direct Support Professional Near Jenkintown, PA
Essential Job Functions/Responsibilities
- Performs simple procedures according to Individual Service Plan
- Assists individuals with daily living skills and community skills
- Utilizes Person Centered Practices to assess individual needs
- Completes training with individuals towards achieving ISP goals
- Performs personal care activities, including but not limited to:
- Bathing
- Shampooing/hair care
- Skin care/nail care
- Oral hygiene
- Shaving
- Dressing
- Toileting/elimination
- Catheter care
- Provides support in areas of independent/home-living such as:
- Meal preparation/feeding/fluid intake
- Provides assistance in in the individuals laundry
- Light housekeeping in areas directly related to the individual’s living area
- Shopping as related to the individual’s community needs and preferences
- Reports any observed or reported changes in the individual’s condition and/or needs to the Executive Director or designee
- Documents care provided and completes daily progress notes. Completes the appropriate records to document care provided. Submits documentation at least weekly
- Promotes personal safety and a safe environment for individuals by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Executive Director or designee
- Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs
- Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested
- Maintains confidentiality in all aspects of the job
- Attends office required programs to meet compliance requirements. Identifies learning needs to supervisor
- Performs other related duties and responsibilities as deemed necessary
Salary: $16.00 - $18.00 per hour
