Cornerstone Case Manager
360 Youth Services, a non-profit agency transforming the lives of youth for over 45 years, is looking to hire a Cornerstone Case Manager
All of the job duties and responsibilities within this document will be performed in accordance with established policy and procedure guidelines within the department and agency.
SUMMARY OF RESPONSABILITIES
Provide program services to residents and be responsible for client staffing, client transitions, program liaison with DCFS, program liaison with court services, client skill development and all other duties as assigned by the Program Manager.
EDUCATIONAL/EXPERIENCE REQUIREMENTS
A Bachelor's degree in social work or a related human services field and 2 years of equivalent work experience. Have appropriate educational background from an accredited school of social work or related field.
Experience with DCFS protocols and policy. Minimum age of 23. Demonstrates an ability and willingness to provide agency designated services in an independent fashion. A valid driver’s license. Demonstrate strong interpersonal skills in relating to youth and staff in a positive, constructive and therapeutic manner. Demonstrate leadership skills which will ensure healthy communication with staff members.
KNOWLEDGE AND ABILITY
Demonstrate an ability to work with youth, families, and social service agencies in a professional manner as an agent for positive change. A commitment to the values and ethics of a professional social worker. Demonstrate at all times a level of personal and professional integrity, honesty and competency that brings credit to the profession and the agency.
Coordinate and lead Client Staffing’s to include but not limited to:
a. Attending Staffing’s for incoming clients.
b. Facilitating Staffing’s for all current clients.
c. Completion of Staffing reports within the required time period.
d. Attend Staffing’s for recently discharged clients.
e. Participation in DCFS Staffing conference calls.
2. Coordinate and organize all discharges of clients to include but not limited to:
a. Completion of Centralized Matching Team Referrals
b. Completion of Discharge Summaries
c. Coordination of Transition Support Services for discharged clients
3. Work with clients to locate and engage family members and adult supports.
4. Set visitation logistics to insure adequate transportation needs are met.
5. Manage client medication supply and administration including submission of psychotropic consents to DCFS as needed.
6. Set medical and dental appointments as needed or required by DCFS.
7. Maintain in-house documents to include; client information sheet, SER/UIR contacts, and medical service reports.
8. Submission of all ACR document needs to DCFS Caseworker.
9. Interface with Residential Therapist to insure thorough communication with Psychiatric Nurse.
10. Develop positive post-secondary education plans (including FAFSA completion, college visits, financial resources).
11. Maintain positive relationships with local area employers and work with clients to secure and maintain employment.
12. Provide administrative backup as needed.
13. Ensure all residents’ charts are complete and contain the required paperwork.
14. Complete and submit all State (i.e. DCFS) required documentation by the necessary time line (i.e. CANS).
15. Participate in all agency related meetings and activities as designated by the Program Manager.
16. All other duties as assigned by the Group Home Program Manager
