Job Summary Provides case management, including client advocacy, education, benefits assessment and enrollment, and coordination of housing support services. Provides eligible supportive services through the Housing Opportunities for People with AIDS (HOPWA) Case Management program administered through the Division of Infectious Diseases. Accountabilities Performs comprehensive case management functions in support of housing support services. Conducts initial intake and assessment Develops and continuously monitors housing and case management service plan. Provides information on housing options and assists with housing location assistance. Provides referrals to ensure clients are receiving medical care. Maintains accurate and current records in Provide Enterprise database system, including documentation of HIV condition, HOPWA assessment, demographics, progress logs, services plans, and services provided. Supervisory/Management Responsibility This is a non-management job that will report to a supervisor, manager, director, or executive. Minimum Requirements Bachelor’s degree in Social Work or in related field; Master’s degree preferred One (1) year of experience in health or social services related area. Experience working with homeless individuals, or people living with disabling conditions, including severe and persistent mental illness or HIV/AIDS preferred Required Certifications/Registrations/Licenses None. Licensed Bachelor of Social Work (LBSW) preferred. In Lieu Of The Above Minimum Requirements N/A Other Required Skills and Experience Psychosocial intervention skills.
