1st Americare is seeking an LPN to work with our pediatric client in Paterson as a home health nurse.

Schedule: Friday, Saturday, Sunday Day Shift

Per Diem / PRN opportunities available.

Duties and Responsibilities:

- Provide skilled nursing care to patients in their homes

- Conduct comprehensive assessments of patients' health status

- Develop and implement individualized care plans

- Administer medications and treatments as prescribed by physicians

- Monitor and evaluate patients' progress and adjust care plans accordingly

- Educate patients and their families on managing their conditions and promoting wellness

- Collaborate with other healthcare professionals to coordinate patient care

- Document patient care activities accurately and timely

Experience:

- Minimum of 2 years of experience as a Licensed Practical Nurse in a home health setting

- Experience with tube feeding and managing enteral nutrition

-Experience with Collar Trach, no vent required

- BLS

- Experience working with children in a healthcare setting
