Overview:
We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will provide high-quality care to patients in the comfort of their own homes. Duties:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patients' vital signs and observe for any changes in their condition
- Provide wound care, including dressing changes and monitoring healing progress
- Educate patients and their families on managing chronic conditions, medication management, and self-care techniques
- Collaborate with interdisciplinary healthcare team members to coordinate patient care
- Document patient assessments, interventions, and outcomes accurately and timely
- Ensure compliance with all regulatory guidelines and standards

Experience:
To be successful in this role, you should have the following qualifications:
- Current RN license in the state of VA or have compact state license
- Minimum of 3 years of nursing experience, preferably in home health or a related field
- Experience in ER, Dementia care, Acute pain management, Epic, Assisted living, Medical terminology, Diagnostic evaluation, Home care, DME is highly desirable
- Strong clinical assessment skills and ability to make sound judgments
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
- Ability to work independently and manage time efficiently
