163rd Street Improvement Council, Inc. is Hiring a Care Coordinator Near Bronx, NY

The Role:

Under the direction of the Case Manager Supervisor, the Care Coordinator is responsible for providing case management services, conducting intakes, initial skills assessments, providing case management, and offering referrals for permanent housing placements or other appropriate programs.

Specific Requirements:

This is a full-time shift position between the hours of 9am-5pm, 10am-6pmng requirement of the next shift be inadequate. In general, it may be necessary, at times, for you to adjust your work schedule in accordance with the operational and staffing needs of the program. Staff may be asked to adjust their work schedule depending on the operational needs of the site.

Duties and Responsibilities:

  • Screen potential clients for program enrollment and orients clients to the program and describes program objectives.
  • Work closely with Residential Aides and Social Workers
  • Research and connect residents to culturally appropriate resources in the community.
  • Assist residents with managing benefits and housing appointments.
  • Maintains contacts with outside agencies to refer or obtain additional client services.
  • Collaborate with service providers to maximize service delivery
  • Conduct initial intake assessments.
  • Utilize assessment tools (CARES) to identify support service needs and makes immediate referrals.
  • Provide the client with Independent Living Plan (ILP) (CARES), working in collaboration with other Social Service providers and ensuring the delivery of comprehensive support services.
  • Track, monitor and document resident progress. Oversees weekly follow-up via one-on-one counseling sessions to address issues, challenges, and successes to strengthen the client’s capacity for long-term self-sufficiency.
  • Maintains confidential case records for all clients.
  • Refers participants to job development for subsequent employment if appropriate.
  • Facilitates workshops addressing issues related to shelter transition including parenting, substance abuse, trauma-informed care, healthy living, Cognitive issues/skills, and Job Skills Training if applicable.
  • Participates in ongoing education and professional development through courses, seminars, and other in-service training in the areas of services for the homeless, cultural diversity training, supportive counseling, and workforce
  • Provides monthly service outcomes information and supporting documentation to Case Manager Supervisor for reporting purposes as required by funding agencies.
  • Performs all other duties as requested by managerial staff and other duties as required.

Essential Skills and Qualifications:

  • Bachelor’s degree in social work, social services or public administration preferred
  • 2-4 years of DHS/HRA and/or other housing/shelter case management experience.
  • Combined education and years of prior experience may be acceptable in lieu of Bachelor’s Degree.
  • Good writing skills are required. Some customer service and job readiness experience preferred.
  • Knowledge of at least one of the following service areas is required: homeless-related issues, mental health, criminal justice, HIV/AIDS, and substance abuse.
  • Prior training experience in the CARES database is helpful. Knowledge of MS Word, PowerPoint, and other database programs.

Job Type: Full-time

Pay: $42,060.89 - $50,000.00 per year
