Job Overview The Licensed Vocational Nurse (LVN) assesses and monitors clients and provides care in accordance with treatment objectives and as instructed by supervisor. Key Responsibilities Participate in the assessment, planning, implementation and evaluation of the nursing care plan. Oversee the ongoing monitoring and evaluation of clients admitted for Residential and Withdrawal Management programs. Provide timely updates and status reports to the Prototypes physician for clients receiving withdrawal management, MAT and/or with co-occurring mental disorders. Facilitate referrals to outside medical providers/facilities for medical clearance, TB tests or treatment when indicated. Facilitate filling of client prescriptions, and work with pharmacy to ensure medications are delivered in a timely manner. Audit medication room and destroy medications as needed per policy. Monitor client care and treatment. Adhere to professional standards. Document client treatment progress, incident reports and medical follow up. Maintain a safe environment for clients. Identify, report and assist with the management of emergencies, and submit Incident Reports to the Program Director in a timely manner. Interact with clients to support their participation in the program. Assist with orientation, admission, transfer and discharge of clients. Provide clerical support as directed by Program Director. Maintain medication book for compliance with all funder requirements and update monthly. Take and record accurate vital signs. Observe universal precautions, including correct handling of blood borne pathogens, storage of lab specimens, and disposal of infectious wastes. Assess and report client behavioral changes. Provide staff training on Medication protocols and Withdrawal Management protocols as needed, including vital signs, observations and documentation. Maintain professional boundaries and standards. Treat all clients with respect, care and concern. Recognize personal biases working within diverse populations. Maintain cultural sensitivity at all times. Demonstrate predictable, reliable, and timely attendance. Follow written and verbal directions and complete assigned tasks on schedule. Read, write & communicate in English and understand basic math. Learn from directions, observations, and mistakes, and apply procedures using good judgment. Work independently or as part of a team; ability to interact appropriately with others. Work with supervision, receiving instructions/feedback, coaching/counseling and/or action/discipline. Influence and improve operations which are not under direct control. Use problem solving methods to continuously improve operations. Education and Knowledge, Skills and Abilities High school diploma or equivalency. LVN, LPT or CNA license required. Experience working with dual diagnosis populations. Good time management. Ability to work in a fast paced environment. Bilingual language skill preferred, English and the predominant other language natural to the community where the facility is located. Good written and verbal skills. Strong Computer skills with experience in EHRS. Dependable automobile and insurance, registration and valid California Drivers License. Knowledge and respect of all confidentiality issues.
