Job Overview The HealthRIGHT 360 - Case Manager (In Custody and Community) Bilingual provides comprehensive case management services for male inmates in Solano County. Case Managers provide information, resources, support and follow-up services as needed and as identified in the Case and Re-Entry Plan. Case management for in-custody clients is an information, recourse and referral driven program. Services include case management, in-custody facilitated support groups, linking clients to services, providing clients community resources and linkage to probation/parole or other community stakeholders. The Case Manager (Bilingual) provides comprehensive case management services for inmates who speak both English and Spanish in the Solano County Detention Facilities. Case Managers will provide information, support and follow-up services as needed and as identified in the Case/Re-entry Plan. Case Managers will also provide in-custody facilitated support groups in the area of cognitive behavior. Case Managers will address needs including but not limited to sober living and transitional housing, stable housing, employment and/or (re)establishment of entitlement benefits, family reunification and counseling, mental health and SUD treatment and ongoing recovery, rent subsidies, deposit assistance, civil legal services, gender specific mentoring, clean slate services, and subsidized employment and transportation to appointments. The Case Manager (Bilingual) will also provide Post-Release case management services linking clients with the needed substance abuse, mental health or health care treatment services which may include; outpatient substance use disorder treatment; medicated assisted treatment; individual therapy; rehabilitation groups, and/or health care visits with a goal to provide assistance with life skills development and on-going support in the community (based on participant need) for 3-9 months after release. Available shift is 8-hour shift - 7:00am - 3:30pm. Monday - Friday - No weekends. Key ResponsibilitiesThe Case Manager is responsible for providing a case management approach that will be assessment-driven, collaborative, strengths based, gender responsive, and client centered. Responsibilities include but is not limited to: Conduct intakes and assessments. Work with clients to create, track, revise, and update client focused Individual Re-Entry Plan drawing on an integrated array of public, private, and project-specific resources.Partners with the County’s Housing Navigators to ensure that all homeless clients receive immediate, individualized housing-related support, including accelerated access to a shelter bed.Following initial re-entry plan development, the case manager will meet with each client every two weeks at minimum. On a case-by-case basis, more frequent contact will be arranged to address emerging needs or client crisis. Responds to inmate request as needed or assigned by Program Supervisor.Responsible for tracking outcomes and providing regular reporting to Program Supervisor and as requested. Case Managers will follow clients during their entire SUD Subsidized Housing episode, assisting with transition as clients “Step Down” to different levels of care.Education and Knowledge, Skills and AbilitiesDesired attributes, skills and experience: Bilingual (English/Spanish) – direct experience working with culturally diverse populations.High school diploma or equivalent. Preferably a minimum of two (2) years of verifiable case management experience providing services for justice involved; homeless, or substance abuse population. Two (2) years of verifiable experience providing CBT services and support groups to the criminal justice population preferred. Familiar with Solano County community providers specifically in the area of those serving individuals incarcerated and also released from incarceration. Training and practical experience in core premises and practices such as motivational interviewing, gender specific issues, trauma-informed care, harm reduction, boundaries, and confidentiality, and stages of change theory. Strong oral and written communication skills; clearly and thoughtfully communicates with others in person and on the phone. Teamwork: Collaborates positively with other team members; giving and accepting constructive criticism and assisting as needed throughout the program. Tolerance for Stress, Ambiguity, and Change: Maintains composure even while under great pressure; handles complex problems and change with minimal supervision; demonstrates flexibility and versatility in achieving key goals and priorities. Being flexible with the day-to-day changes.Decision Making: Collects, organizes, and analyzes information before making decisions. Takes a thoughtful approach when considering options; may seek supervision and/or input from others. Integrity and Ethics: Actively models the highest ethical standards, is honest, accountable, maintains confidentiality and appropriate boundaries at all times, handles sensitive information and issues with discretion and tact. Strong proficiency with Microsoft Office applications (Excel, Outlook and Word) and internet applications.Cultural Sensitivity: Willingly open to learn and understand different perspectives.Education and Knowledge, Skills and Abilities Required: Ability to express empathy, compassion and communicate honestly. Knowledge of clinical documentation (treatment plans, progress notes etc.). Experience working with issues of substance abuse, mental health, criminal background and other barriers to economic self-sufficiency. Experience delivering evidence-based practices. Experience delivering curriculum-based cognitive support groups. Experience working with criminal justice population. Bilingual (English/Spanish) speaking (encouraged to apply) language differential pay available. Background Clearance: Valid California Driver’s License, reliable vehicle, and willingness to travel locally. Must undergo a background check to be cleared to work for the agency and in Solano County Jails. Must not be on active parole or probation. #ZR
