This is a Limited Duration Assignment (LDA), which means this position can last up to two years, but may end at any time. At Corrections Health, we provide medical, dental, mental health and transitional support services for Adults and Youth in Multnomah County MCSO or DCJ custody. Mental Health Associate LDA - Is a care coordinator who is jail-based, to address the mental health, care, and overall safety of complex individuals in custody at multiple correctional facilities in Multnomah County. The Mental Health Associate LDA works alongside Qualified Mental Health Professionals, Nurses, Providers and other staff including correctional staff (Multnomah County Sheriff's Office) within a healthcare environment. This position works with clients on-site via a caseload by providing services not limited to: Direct support Needs assessment Service connection Benefit and services eligibility determinations Housing and employment Connection to legal representation Education and skills training The Mental Health Associate LDA will facilitate coordination with multiple internal and external partners including the courts, law enforcement, and other departments and divisions, focusing primarily on advocating for individuals with severe mental illness who may present a risk of harm to themselves or others. Duties of the Mental Health Associate LDA included but are not limited to the following which is a representative sample of the level of work appropriate for position: Engagement, Assessment, Client Monitoring and Support The Mental Health Associate LDA will engage and build rapport with individuals who may be experiencing active psychosis, depression, anxiety and other potentially severe symptoms of mental illness often complicated by substance abuse, trauma, poverty, and significant medical issues. Maintain a complex caseload Assist in the implementation of behavioral support interventions Provide skills training and conduct assessments Develop and reinforce safety plans Determine eligibility for services Participate in crisis interventions at the appropriate level as instructed by a Mental Health Consultant or other Provider Process release of information requests (ROIs), mental health requests, and other communications Keep all records in accordance with HIPAA, timely and complete Teamwork and Coordination This position will participate in meetings, retreats, and case staffing's. The Mental Health Associate LDA will communicate with the assigned Mental Health Consultant and other team members to coordinate case management activities, to ensure service plans are in line with other treatment goals. Report client outcomes and progress to the team Attend meetings and participate in indicated workflows and processes Facilitate psychoeducation groups and/or other groups Conference client concerns with the team for coordinated care Seek out team review for risk response for planning housing and legal support connections Document in case files and record Suggested services and actions needed as part of the transition plan at release or to the next level of care Act as an advocate for the client with external agencies eg. insurance and medical systems etc. Research and follow up with external providers and services to verify attendance, actions completed and risk concerns for the client Community Outreach This position assists with outreach to internal and external partners, community services, providers, crisis lines, etc., to provide program information, and to support existing and new connections to meet clients' needs. Review client needs and case file for internal and external support Collaborate with team members in the Corrections Health Transition Services Program or other partners to communicate client's case and care plans for both short and long-term goals Serve as member on internal/external committees and workgroups for representation of Corrections Health Mental Health Services. TO QUALIFY: We will consider any combination of relevant work experience, volunteering, education, and transferable skills as qualifying unless an item or section is labeled required. Minimum Qualifications/Transferable Skills*: Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. Education/Certification: Must meet the standards of Qualified Mental Health Associate - (QMHA) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at MHACBO website. QMHA within 30 days of hire, current QMHA, and/or licensed Board of Oregon (LPC, LCSW, PhD,etc) Graduate degree in psychology; Graduate degree in social work; Graduate degree in a behavioral science field; Current CPR/BLS certification (Valid certification upon hire) May require a valid drivers’ license. Experience working in a clinical setting Experience as a case manager or transition planner Experience in a corrections setting working with a diverse population who display mental health and behavioral issues. Licenses/Certificates: Certification in Alcohol and Drug Counseling is highly desirable.
