Department/Unit: Medicine Nutrition Work Shift: Day (United States of America) Case Manager Eating Disorders Program Albany, NY As a Northeast Comprehensive Care Center for Eating Disorders (NECCCED)for eating disorders the case manager is a liaison for patients and families to facilitate seamless transition of services and ensure timely, effective treatment interventions. Care manager ensures necessary linkages, advocacy and follow-up are provided for NECCCED patients. Duties include assessment and screening, crisis management, referral to emergency services when necessary, linking individuals and families to psychiatric, psychological, medical, educational, social, vocational, and other essential services, developing strategies for implementing treatment plans, and monitoring patient adherence to recommendations. Essential Duties and Responsibilities Conducts age appropriate interviewing with clients identifying problems such as adjustment to illness, employment, food, clothing, housing, substance abuse, and physical and mental impairments to determine nature and degree of problem. Secures information such as medical, psychological, and social factors contributing to client's situation, as well as their annual income, and evaluates these in the context of the client's capacities. Assesses persons in crisis and provides intervention, keeping self and client safety as principle objective of contract, with an understanding of legal responsibilities in cases of domestic violence and abuse issues. Facilitates the utilization of resources to meet patient outcomes (case management). Analyzes patient care needs and utilizes a systematic approach to delivery of care. Performs telephone triage including coordination of care and documentation. Works with physicians and other resources to develop teaching tools for families and patients. Age appropriately counsels client individually and /or with significant social supports regarding plans for meeting needs, aids the client to mobilize inner strengths and works cooperatively with community resources to improve overall functioning and independence. Counsels patients in adjustment to life threatening illness and its implications, acknowledging issues of diversity and adjusting intervention and practice accordingly. Understands legal issues around domestic violence and abuse issues, and intervenes appropriately and with accordance of Albany Medical Center’s policy and NYS law. Provides patient and family education taking into consideration cultural, religious, language, and motivational, physical, emotional and cognitive factors into account in the teaching plan Works with other departments inside and outside of AMC to coordinate multidisciplinary care Utilizes educational resources on Intranet for self education and education of other staff and families Works in close collaboration with physicians and other health personnel in client evaluation and treatment to further their understanding of significant social and emotional factors underlying client's health problem. Facilitates formal case conferences, as needed, to provide coordinated services to the patient. Screens client for eligibility for entitlement programs and refers client to appropriate services/agencies. Refers clients to other community resources and other organizations based on identified needs. Accesses and records client and community resource information. Education and Experience Bachelor's degree in Psychology, Sociology or Social work required, Masters Degree preferred; 1 to 3 years documented experience in the provision of case management services to medically compromised patient population, familiarity with community resources, and ability to work as a member of al interdisciplinary medical team.
