The Community
Our beautiful Residential Care Facility is located in the desirable location of Southwest Portland. We are minutes away from downtown, Tigard, and Lake Oswego. We are a 32-bed community that is split amongst two cottages. We promote ourselves as a community that follows the ‘Household Model’ – We empower our staff, create a home-like environment, and follow a person-centered care approach.
Care Coordinator Overview
We are looking for someone who works well in teams and engage with staff daily. You will be a crucial part of our management team that will involve the Administrator, Assistant Administrator, and Health & Wellness Director (RN). This management team will be responsible for daily community oversight and compliance.
As the Care Coordinator, you will be working closely with the community LPN/RN to develop resident evaluations and Service Plans. The RCC is normally the first point of contact regarding clinical care on a daily basis, if significant change occurs – You and the LPN/RN develop a new plan approach.
Medication management and systems - will be an essential part of your job. Our Med Aides rely on our Care Coordinator for guidance when unforeseen issues may arise.
Pharmaceutical Relations – Our Med aides are responsible for re-ordering medications. The RCC makes sure that these meds are ordered effectively and in a timely manner.
Staff Development – Our small community works together to teach, train, and develop staff to become the best care partners they can be. The RCC will work alongside the Administrator and Assistant Administrator to hire and train new staff.
