Job Responsibilities:

• Schedule and perform regular home and field visits and face-to-face contacts
• Conduct comprehensive assessments
• Develop service plans in collaboration with recipient and with the implementation of specific goals and objectives based on the identified needs of the assessment
• Serves as advocate and liaison between recipient and community agencies, programs and resources
• Record every activity, service or intervention on a daily basis via Progress Notes
• Prepare progress reports and court reports (if applicable)
• Attend court hearings related to the recipient (if applicable)
• Communicate with psychiatrist, primary care physician and other professionals working with the recipient maintaining a collaborative relationship
• Attend supervision, trainings and staff meetings as scheduled
• Comply with productivity guidelines and submit weekly billing reports on a timely manner
• Share all concerns with Supervisor, Executive Director or Designee.
• Prepare a termination summary prior to the recipient ge from the program
• Have knowledge of available resources within the community
• Reporting Relationship:
• This person reports directly to the Case Management Supervisor
• Qualifications:
• Must have a bachelor's degree (B.A. or B.S.) from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or in any related social services or behavioral health field;
• A minimum of two (2) year of direct experience working with people with mental health conditions and emotional disabilities;
• Must have the AHCA-approved TCM Certificate.
• Must be certified by the Florida Certification Board (FCB) as Certified Behavioral Health Case Manager (CBHCM) or must have the provisional certification (CBHCM-P)
• Requirements:
• A valid Florida driver's license, insurance, reliable transportation and be capable of unrestricted travel in the performance of his/her duties.
• Basic computer skills, fluency in English and Spanish preferred
• Strong organizational, time management and data management skills
• Knowledge of community resources
• Awareness of the social, economic and cultural factors that affect families in our community
